The patients enrolled must be over 65 and receive outpatient services at least four times per year at this medical center (diagnostics, radiology, labs, physical therapy, etc.). Care coordinators help act as a hub of services, assisting the patient in managing appointments, scheduling, medication, and communication with providers. This study is one year and will assess patients’ satisfaction with the services and health outcomes as compared with a “treatment as usual” group.
