Part 2 – Narrative

Justification of Concept Map Components

Medication, diet changes, and physical exercises for diabetes are effective methods of managing the disease’s effects. The main condition get-
ting assessed is type 2 diabetes and its adverse effects. Healthcare outcomes are dependent on the behavior of diabetes patients to en-
sure they receive care that improves their mental and physical wellbeing. Goyal & Jialal (2021) stated that diabetes does not have any cure;

thus, it requires proper management that ensures an effective method of limiting the adverse effects. In the concept map, three diag-
noses were selected that are Type 2 Diabetes, pregnancy, or Polycystic ovary syndrome. After an accurate diagnosis, persons with type di-
abetes can encounter issues whereby it is imperative to manage personal conditions and adhere to medication to limit critical situations. Man-
agement of socio-economic factors is involved in diabetes management since they determine if the patients would be able to afford care in
hospitals or home-based care. Health-related challenges of diabetes usually occur when the persons do not adhere to proposed medication
plans. When someone gets diagnosed with type 2 diabetes, they can fail to adhere to management provisions that include proper lifestyle con-
siderations. It is imperative for patients who do not have the desire to manage their conditions to get educated on the dangerous effects of the
condition. The treatment considerations and outcomes listed in the concept map, lack of knowledge of proper management, and improper ad-

herence to health guidelines should be managed to prevent adverse health effects. Interprofessional Strategies for the Concept Map

Integration of treatment measures is applicable to analyze health organizations more capable of dealing with diabetes management. Ex-
perienced physicians and nurses are involved in managing diabetes since they are provided with many resources that limit the disease’s adver-
sity. Research reveals that many persons with type 2 diabetes do not adhere to medication (Reed et al., 2021). Medication, diet changes, and
physical exercises for diabetes are effective methods of managing the condition. During primary care, patient outcomes depend on patients’

behaviour to ensure they accept care that improves their mental and physical wellbeing. Protection of patients is possible by handling
their medical data and frequent glucose monitoring readings. This shall result in better health outcomes in connection to all healthcare provi-

sions.

Training providers to integrate better care shall be possible to facilitate reliable health outcomes. Training sessions would improve patient
outcomes based on the availability of personal acceptance of diabetes medication and management measures. Collaboration among physi-
cians, nurses, and therapists can improve the accuracy of different health measures (Bellou et al., 2018). The affected persons manage their
conditions as physicians or nurses monitor them. Health system factors can affect the validity of care since adherence to medical informa-

tion requires proper medication, involvement of healthcare providers, and adherence of patients. References
